As patients’ access to health data, information, and tools continues to increase, patients can monitor their own health and engage their clinicians in informed discussions about their care. Medical professionals can better support patient decision making both within and beyond clinical encounters through trustworthy digital tools to support collaboration between patients and their clinician through shared decision making (SDM), improved integration of patient-contributed data into patient-centered clinical decision support (PC CDS), and the integration of PC CDS within the phases of SDM.

About the Report

This report is based on a scoping review of peer-reviewed and grey literature as well as key informant interviews to identify relevant models of SDM and examples of the application of PC CDS in SDM. Results from these activities informed the development of the PC CDS-SDM Framework.

The framework builds on the Three Talk Model which outlines three phases of SDM: team talk, option talk, and decision talk. The framework integrates the three phases of SDM with three elements of PC CDS:

Knowledge (e.g., patient-centered outcomes research),
Data (e.g., patient-reported outcomes [PROs], social determinants of health [SDOH]),
Delivery (e.g., access and engagement via applications, portals) to showcase how PC CDS can support SDM.

How to Use the Report

This report addresses the knowledge gap between how PC CDS can support SDM by providing a framework for the use of PC CDS tools in SDM. The information in this report supports clinicians and healthcare systems aiming to enhance SDM through the incorporation of PC CDS, as well as the use of patient preference and other patient-specific information at the point of care, and can be used by developers of PC CDS and electronic health record systems.
